Course Description

In this program participants will be learning Enhanced Care Coordination (ECC), including effective interdisciplinary teaming, engagement techniques (motivational interviewing), how to plan and collaborate with families or caregivers, effective planning with adolescents/youth, goal setting, benchmarking using data to continuously monitor progress, advocacy and negotiating strategies, facilitating meetings with providers and natural supports, community outreach, and developing collaborative relationships with community providers. Trainees will also learn how to support transition-age youth using youth-driven person-centered planning, career development, issues related to benefits changes, and post-secondary education and training programs

The program will support learners to: 

  • Understand the values and principles that guide care coordination.
  • Understand an individuals' role in care coordination.
  • Employ a trauma-based approach working with children and youth with difficult behaviors.
  • Develop and apply skills in connecting and building trust with children and youth.
  • Apply skills to help children and regulate and manage their stress.

Program learning will take place in weekly online seminars and asynchronous online instruction.  Course materials will include a vast array of articles, rich-media, handouts, discussion activities, didactic lecture notes, presentations, assessments, and field relevant resources.
